DESCRIPTION OF WORK
As a Social Worker, you will serve a pivotal function in facilitating the successful community reintegration of individuals managing mental health conditions, thereby enabling them to lead meaningful and fulfilling lives. You will collaborate effectively with an interdisciplinary team, including legal professionals and treatment providers, to optimize consumer care and support services. Key responsibilities will include managing and orchestrating communication, scheduling and participating in meetings, and arranging or providing transportation for consumers during discharge or follow-up planning processes. Through your professional endeavors, you will directly contribute to enhancing the well-being and supporting the recovery processes of those under your care.
