Case Manager RN, Case Management PRN
Description
The Case Manager strives for excellence in patient care by building a collaborative caring work environment coordinating care and information across the various disciplines and levels of care while focused on patient care outcomes and wellness.
The Case Manager assesses, plans, facilitates Care coordination and services, and continually re-evaluates agreed upon plan of care to best meet the patients’ healthcare needs across the continuum of care.
The Case Manager sets goals of care with the patient, family, and / or Care-giver. The Case Manager advocates for the patient through communication and evaluation of available resources to promote quality cost-effective and safe outcomes.
The Case Manager incorporates standards of care for Case Management including identification / selection of patients for case management services, problem identification, planning, monitoring, evaluating, transition of care and outcomes.
The Case Manager implements practice based on current research and evidence based practice and ensures adherence to hospital and department policies, regulatory standards, and guidelines.
The Case Manager supports an environment of caring focused on the patient’s healing and wellness.
Minimum Education, Experience, Training, and Licensures Required :
- Bachelor of Science in Nursing (BSN)
- Current State of Kentucky RN license
- Basic Life Support (BLS)
- RN with five (5) years’ experience in acute care nursing, case management, utilization management or related field.
