Comment(s): The HFH Interim Housing (IH) Clinical Services team Registered Nurses provides critical clinical subject
matter expertise and support to contracted IH providers addressing and supporting the medical and behavioral health needs
of participants at IH recuperative care and stabilization sites. IH staff also support with the participants Transitions of Care
(TOC) post-hospital discharge. This is done through completion and review of participants referrals and assessments, care
plan development and coordination, client/facility in person visits, ongoing evaluations, and monitoring of participants cases
in collaboration with an interdisciplinary team. RN will assess support services and provide training and technical assistance
to IH providers engaged in long term care of their clients.
