Job Title : Medical Case Manager
Location : Augusta , ME
Duties :
- Shall track medical status of all assigned Soldiers.
- Schedule profile exams / evaluations and audiology exams at periodic health exam facility.
- Coordinate Soldier treatment plans with appropriate medical facilities.
- Correspond with soldiers to obtain information for the Soldier’s case management file, referred by a provider are followed.
- Manage Soldier’s care from the identification of the condition to when the Soldier is either considered medically ready and returned to duty or administratively separated from the military.
- Educate Soldiers on resources available to improve medical and dental readiness.
- Review and track health assessment lab results and contact SM and educate on lifestyle modifications.
- Update MODS as required on the following : immunizations, designation of pregnancy, eye exams, and medical warning tags.
- Track medical appointments by communication with unit to ensure Soldier attendance and submission of follow up documentation.
- Shall prepare medical documentation for Line of Duty (LOD) investigation for units for which they are responsible in conjunction with Health Service Support (HSS).
- Shall mail all medical records to ensure timely and accurate processing of medical records to and from mobilization sites, 30 calendar days prior to soldiers’ report date.
- Shall enter individual vouchers for SM IMR requirements into the Automated Voucher System (AVS) when a soldier misses a MRE or SRP.
Qualifications :
- Shall have at least 5 years of case management experience
- Minimum of 3 years clinical experience.
Education / Certification :
- Registered Nurse III (RN) License
- Preferred Certified Case Manager (CCM) Certification.
