Program Overview : Henry Street Settlement Older Adult Center is a multilingual and multicultural program that has served the needs of the Lower East Side’s senior community since 1952 by offering a variety of activities and services. The Center provides two nutritious and affordable hot meals daily, Sunday through Friday. Seniors are encouraged to participate in activities promoting health and wellness, physical exercise, recreation, education, technology, and arts & culture, and to take advantage of fitness rooms complete with exercise machines and free weights.
Qualifications :
- Bachelor’s Degree required with 1 years of social services experience required
- Fluent bi-lingual in Mandarin / English required
- Strong Microsoft Office skills (primarily Word and Excel)
- Good verbal and written communication skills
- Possess the flexibility, empathy, and compassion to work with the senior population and diverse cultures
- Possess case management and entitlement & benefit knowledge and / or experience
- Some knowledge of mental health issues among senior populations required
- Possess a willingness to learn, a positive attitude, and be a team player
Responsibilities :
Respond to walk-in clients in need of case management / assistance, conduct intake & bio-psycho-social assessments, and coordinate appropriate services and benefits for clientsMonitor and assist clients in coordinating medical, legal, and financial issuesIdentify benefit and entitlement programs that the client should be eligible for; assist the client in applying for those benefits and advocate for them should they be wrongly denied themIdentify and link members who may need assistance and / or referral to other Henry Street Settlement servicesProvide information and referral for clients to appropriate community servicesDocument all client interactions in an ongoing work log and in the Department for the Aging database (STARS)Plan workload to meet contractual service levels and the needs of the program & its membersDevelop and lead a member support groupProvide task supervision to BSW and MSW internsComplete and submit monthly statistical reportsAssist with activities and meal service when neededOther duties as assigned by supervisor(s)Equal Opportunity Employer
