Michigan Licensed Senior Care Manager

A company is looking for a Senior Care Manager (RN).

Key Responsibilities

Develops and assesses ongoing care / service plans while collaborating with providers to address member needs

Coordinates care between members, families, and providers to ensure appropriate person-centered services

Monitors member status and revises care plans as necessary to meet goals and address unmet needs

Required Qualifications

Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing

4 - 6 years of related experience

Registered Nurse (RN) state licensure and / or compact state licensure required
