Job Summary :
Coordinates and delivers patient care using the health care team philosophy practice model. Coordinates activities with ancillary departments. Maintains standards of department and the hospital. Reports to Administrative Director of Critical Care Services. Facilitate or initiate care of patient with sudden or gradual decline in patient condition. Orders are directed and approved by the Medical Executive Committee.
Performance Qualifications
- Respond and assist with all Code Blues, Rapid Response Calls, In-house STEMIs, Stroke Alerts
- Respond to concerns of the RN staff about patients experiencing unexpected status changes
- Facilitate communication and action among the healthcare team to best meet the needs of the patient
- Initiate or assist with interventions to stabilize patient
- Expedite transfer of patient to higher level of care as appropriate
- Make rounds on all patient transfers from ICU within 12 to 24 hours post transfer
- Complete a post RRT follow-up with the patient within 12 hours of the rapid response being called
- Perform focused nursing assessment on all patients seen
- Observe for clinical trends, lab trends, and early signs of decline, and review plan of care
- Make recommendations to the healthcare team as needed
- Provides support and education to the RN’s on the floor by collaborating and sharing his / her skills regarding patient care in a non-judgmental and non-critical manner
- Provide a complete and accurate hand off using SBAR
- Assist with transport of critical care patients to procedural areas from needed areas (ICU, ED, Inpatient units)
- Provides the best possible nursing care by planning, organizing, and directing the nursing functions of patients on the unit.
- Initiates and implements patient care plan.
- Maintains acceptable standards of patient care.
- Identifies problems and guides personnel to their solution.
- Creates a working climate that provides growth and job satisfaction of personnel.
- Accurately and promptly implements physicians' orders.
- Administers medications and intravenous solutions skillfully and correctly.
- Maintains the standards of accurate and complete recording and reporting.
- Arranges to have needed supplies and equipment on hand.
- Participates in planning changes and improvements.
- Participates in hospital safety programs.
- Supports and enforces infection-control policies and procedures.
- Is resourceful and calm in emergencies.
- Participates in orientation and in-service training for personnel.
- Participates in unit and department performance improvement plans.
- Assists in reviewing and revising policies and procedures.
- Maintains and respects patients' privacy by maintaining confidentiality of all information regarding patients, families, physicians and hospital personnel.
- Displays concern and initiative.
- Observes and supports hospital policy.
- Is prompt and efficient with minimal absences.
- Cooperates and maintains good rapport with nursing staff, medical staff, other departments, and visitors.
- Maintains certification for placement of vascular access devices : PICC lines, midlines, and specialty IV catheters
- Maintains sterility and follows best practice guidelines for insertion and maintenance of all vascular access devices.
Qualifications : Education / Skills
Graduate of a recognized non-online RN program with a Bachelor of Science degree in nursing preferred.Employee must demonstrate ability to recognize patients' individual needs based on medical conditions, age adolescents, young adults, middle-aged and geriatric), limitations and planned procedures. Requires good judgment in delivering patient care; good oral and written communication skills; ability to function in stressful situations; current review in CPR. Must be willing to rotate shifts, days off and work area; may occasionally be required to work overtime.Work Experience :
Critical Care or Emergency RN with 3 to 5 years’ experience preferred.
