Based in Plymouth, MA, as an Aide , you'll play a pivotal role in supporting individuals with disabilities, helping them thrive. You'll be a crucial part of Community Connections' mission to provide optimal independence and enriching services to people with disabilities, making a lasting impact in their lives. We are looking for an Aide with a commitment to compassion, caring for others and serving the community.

What You'll Do as an Aide :

• Assist individuals in developing and refining skills, promoting functional independence during breaks, mealtimes and other specific routines.
• Help to implement DHSP goals within a group setting or a one to one situation
• Assist individuals physically as necessary
• Document individuals progress toward DHSP goals
• Ensure individuals health, safety and general well-being
• Ensure and safeguard the human dignity, rights and liberties of the individuals we serve
