Summaries : Assess patient health problems and needs, develop and implement nursing care plans, and maintain medical records.
Administer nursing care to ill, injured, convalescent, or disabled patients. May advise patients on health maintenance and disease prevention or provide case management .
Duties and Responsibilities :
- Admission process for new members
- Establishing ADCAPS, Care Plan
- Renewing client’s 120 Days ADCAP
- Renewing client’s 180 days CP
- Renewing client’s physician order every 180 days.
- Assessment the members of the time admission.
- Must be able to perform various sensory requirements such as vision, speech, smell, touch, manual dexterity, fine motor skills, and hearing in order to assess and treat the patient accurately.
- Actively demonstrate a working knowledge of nursing theory, techniques, principles, and practice to holistically care for the patient.
- Follow the state requirement when disposing of biohazard material, labeling and handling of all materials.
- Perform physical exams and health histories.
- Administer medications, would care, and numerous other personalized interventions.
- Interpret members’ information and make critical decisions about needed actions.
- Observing and recording members behavior
- Consulting and flowing with the physicians’ orders and other healthcare professionals.
- Coordinate care, in collaboration with a wide array of healthcare professionals
- Direct and supervise care delivered by other healthcare personnel like LPNs, CNAs, and nurse aides.
- Conduct research in support of improved practice and patient outcomes
- Performing diagnostic tests
- Administering medications and treatments.
- Maintain accurate, detailed reports and records.
- Monitor, record and report symptoms and changes in members' conditions
- Record members' medical information and vital signs /
- Modify members' care plan as indicated by members' responses and conditions.
- Consult and coordinate with health care team members to assess, plan, implement and evaluate patient care plans.
- Will handle and be knowledgeable of all medical equipment utilized in the care of the members.
- Will frequently lift up to 100 or more pounds consistently causing a high volume of stooping, bending, lifting, pulling and twisting.
- Monitor all aspects of patient care, including diet and physical activity.
- Order, interpret and evaluate diagnostic tests to identify and assess members' condition.
- Inform physicians of members' conditions
- Comply with all facility policy and procedure, while upholding the high expectation of Maryland Care.
