• Implementation of physician xe2 x80 x99s prescribed plan of care
• Patient assessments and coordination of care xc2 xa0 xc2 xa0 xc2 xa0
• Administration of prescribed medication, treatments, and therapies xc2 xa0 xc2 xa0 xc2 xa0
• Compliant, timely documentation of care provided in system of record xc2 xa0 xc2 xa0 xc2 xa0
• Health promotion, teaching, and training of family members xc2 xa0 xc2 xa0 xc2 xa0
• Responsible for accepting and writing verbal orders from physicians as required and for obtaining co-signature of Case Manager on all verbal orders xc2 xa0
• Follow up with physician, client, family members, staff, and Case Manager on client problems, concerns, and / or changes in client condition xc2 xa0 xc2 xa0 xc2 xa0 xc2 xa0
• Performs other appropriate duties as assigned xc2 xa0

Qualifications xc2 xa0 xc2 xa0 xc2 xa0 xc2 xa0

• Must have and maintain an active, unencumbered license (RN / LPN / LVN) in the state in which the clinician will practice xc2 xa0 xc2 xa0 xc2 xa0
• Current CPR certification from American Red Cross, American Heart Association, or BLS xc2 xa0 xc2 xa0 xc2 xa0
• TB skin test (current within last 12 months) xc2 xa0 xc2 xa0 xc2 xa0
• Six months prior hands-on nursing experience preferred but not required xc2 xa0 xc2 xa0 xc2 xa0
• Must have reliable transportation xc2 xa0 xc2 xa0 xc2 xa0
