Position Overview : We are seeking an experienced Certified Medical Assistant (CMA) with a minimum of 5 years in clinical care to join our Chronic Care Management team. The ideal candidate will be skilled in patient communication, documentation, and coordination of care for patients with multiple chronic conditions.
Key Responsibilities :
Provide telephonic outreach to enrolled CCM patientsAssist in care planning and documentation in compliance with CMS guidelinesCollaborate with providers, specialists, and care teams to ensure continuity of careMaintain accurate and up-to-date patient records in the EMREducate patients on their chronic conditions, medication adherence, and preventive careIdentify and report clinical concerns or changes in patient statusQualifications :
Certified Medical Assistant (CMA) requiredMinimum 5 years of experience in a clinical setting requiredStrong understanding of chronic disease management (e.g., diabetes, hypertension, CHF, COPD)Excellent communication and organizational skillsExperience with electronic medical records (EMR)Prior CCM experience preferred but not requiredWhat we offer :
