Key Responsibilities :

Administer therapy treatments in patients’ homes under PT oversight

Conduct ongoing patient assessments and care team communication

Maintain detailed and timely documentation of services delivered

Educate patients and caregivers on therapy goals and home exercises

Must Have + Certifications :

Valid PT license in Minnesota

Prefer minimum two years’ PTA clinical experience

Current driver’s license, insured vehicle

CPR / BLS certified
