• Experience with computerized documentation is required.
• Skills in care coordination, discharge planning, and patient assessment are essential.
• Familiarity with care coordination and discharge planning for adult and geriatric patients.

Description :

The Case Manager will be responsible for managing a caseload of up to 25 patients.

This includes assessment, care coordination, and discharge planning based on evidence-based guidelines.

Experience with HMOs, IPAs, and understanding of CMS requirements is crucial.

Knowledge of CPT coding, medical necessity determination, and HIPAA guidelines is expected.

The position requires effective communication and collaboration with interdisciplinary teams.

Candidates must be willing to float between facilities as needed.

Weekend coverage is typically required, amounting to one full weekend every four weeks based on departmental needs.

Working on three of the five major holidays is mandatory.

References from two supervisory figures covering the past year are necessary for application evaluation.
