A Community Health Worker (CHW) contributes to AHF by blending the mission with care coordination. The CHW provides preventive health services to prevent disease, disability, and other health conditions or their progression; to prolong life; and promote physical and mental health.
A CHW enhances the reputation of AHF by contributing to improved member health outcomes and advancing health equity, ultimately supporting healthier, and happier lives. The CHW works with members in their homes, communities, or clinic settings to support adherence and compliance to care, while also helping identify and address socio-economic factors that impact overall health.

To be successful as a CHW it is crucial to ensure that the AHF Core Values and Mission stay top of mind with all that you do. At AHF we are nimble and able to adapt in a dynamic environment to assist in providing the best experience for our clients and workplace for our employees.
AHF has a collaborative organizational structure where staff are accountable to multiple leaders.
The CHW will work as a part of a close-knit team to address issues that include, but are not limited to, the control and prevention of chronic conditions or infectious diseases; mental health conditions and substance use disorders; need for preventive services, perinatal health conditions; sexual and reproductive health; environmental and climate-sensitive health issues; oral health; aging; injury; domestic violence; and violence prevention. The CHW proactively assists members by identifying barriers to care and connects them with community resources that support whole-person health. With strong knowledge of available services, the CHW works to address members' physical, emotional, and social needs. The CHW assists with behavioral health referrals and assists with scheduling appointments.
Licenses and Certifications : Community Health Worker Certificate
Managed Care experience preferred.
Bilingual Spanish preferred.
