• Develop and implement strategies to improve patient flow and reduce hospital readmissions.
• Collaborate with the interdisciplinary team to ensure continuity of care across all phases of treatment.
• Ensure compliance with regulatory requirements, accreditation standards, and hospital policies.
• Monitor departmental performance, set quality and financial goals, and report on key performance indicators.
• Recruit, mentor, and develop case management staff to support the delivery of high-quality care.
• Engage with patients and families to address concerns and provide education about care transitions.
