You’ll be the trusted face of our home health and rehab services in the Fort Myers, FL (Lee County) area.

Building relationships, uncovering new referral sources, and ensuring more families have access to high-quality, compassionate care. This is a position for someone who thrives on connections, knows how to grow a territory, and wants their work to make a tangible impact.

What You’ll Do

• Educate providers and the community on in-home rehab and skilled care services
• Grow referral relationships with hospitals, SNFs, assisted / independent living facilities, and physicians
• Track activity and results to stay organized and ahead of goals
• Use market data (Trella) to target high-potential partners and drive smart outreach
• Coordinate closely with internal teams to ensure smooth patient onboarding
