Position : Registered Nurse Per diem New Graduates Welcome Location : Bucks County, PA

- Coordinate total patient care by conducting comprehensive health and psychosocial assessments in the home setting.
- Perform admission, transfer, re-certification, resumption of care, and discharge of the home care patient.
- Provide oversight for non-skilled staff members providing direct care to recipients in the home setting.
- Must be willing to provide in-home nursing care (wound care, infusion services, medication education) to adults and manage patients.
- Role is primarily field based with minimal time spent in the office.
- Must be a graduate from an accredited School of Nursing.
- Must be licensed in the state of Pennsylvania, as a Registered Nurse.
- One, or more, years of experience, in a community / home health agency or in a home setting, is preferred.
- Must have knowledge of Medicare and Medicaid guidelines.
- Must be detail oriented.
- Must have a criminal background check.
- Must have current CPR certification. Online certification is not acceptable.
- Must have a valid driver's license with current active auto insurance.
