Job Title: Social Worker - Case Manager (Adolescent Integrated Behavioral Health)


The Mount Sinai Hospital is a 1,134-bed facility with an extensive outpatient and specialty care network and is the largest hospital in the Mount Sinai Health Care System. It offers comprehensive social work services and programming to meet the myriad needs of the diverse populations we serve. We believe that total patient care must emphasize the physical, emotional, and social needs of each patient and their care partners. Social Workers collaborate within interprofessional teams to serve patients and the larger community from both a micro and macro level including both direct care and prevention. Founded in 1907, the Department of Social Work Services at the Mount Sinai Hospital is one of the oldest hospital social work departments in the nation with over 450 licensed social workers employed across more than 65 different program/service areas. Employment in our department provides a pathway to LCSW licensure as well as access to a broad range of continuing education, professional development and extra-curricular opportunities.

The Mount Sinai Adolescent Health Center (“MSAHC” or the “Center”) works to break down racial, social and economic barriers to health care and wellness for young people by providing vital services - high quality, comprehensive, integrated and confidential - for all who come to the Center or school-based health facilities, at a low cost to them. The MSAHC advances adolescent and young adult health as a national imperative by serving as a leading center of clinical care, specialized training, innovative research and advocacy.

The Social Worker will provide case management for at-risk adolescents (ages 10–26) and their families in an integrated medical and mental health primary care setting at MSAHC. This role emphasizes whole-person care by integrating behavioral, medical, and social support to address mental health, substance use, psychosocial challenges, and health-related social needs that impact patient outcomes. The Social Worker also engages families and caregivers as necessary to support follow-through and healthy adolescent development. Services are trauma-informed and guided by a health equity framework, promoting culturally and developmentally appropriate care for diverse adolescent populations.


Responsibilities
  • Collaborate with primary care providers, behavioral health clinicians, school personnel, psychiatric consultants and community resources.
  • Identify and address social determinants of health (SDOH) affecting adolescents, such as housing instability, food insecurity, transportation, and safety concerns.
  • Develop and maintain individualized care plans addressing medical, behavioral health, and social needs.
  • Coordinate referrals to specialty behavioral health, substance use treatment, and community resources.
  • Monitor progress over time, adjust care plans in collaboration with the primary care team, and promote resilience and healthy coping strategies.
  • Engage caregivers and families in care planning and education, respecting confidentiality and adolescent consent laws.
  • Participate in case reviews, interdisciplinary care team meetings, and quality improvement initiatives.
  • Document assessments, interventions, and outcomes accurately in the EHR.
  • Track follow-up appointments and ensure continuity of care.

Qualifications
  • Master?s degree in social work (NYS LMSW/LCSW).
  • Experience in adolescent behavioral health, case management, primary care, or community health.
  • Excellent engagement, assessment, and collaborative skills, with ability to work flexibly in a high-volume, fast-paced setting.
  • Knowledge of adolescent development, Family engagement, advocacy skills mental health, and SDOH.
  • Strong communication, care coordination, and documentation skills.
  • Knowledge and practice of Motivational Interviewing (MI) to engage adolescents and families, support behavior change, and promote self-efficacy.
  • Experience working with underserved youth populations.
  • Experience in integrated or collaborative care models
  • Bilingual skills (Spanish)

Compensation

The Mount Sinai Health System (MSHS) provides salary ranges that comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $88691.2 - $101670.78 Annually. Actual salaries depend on a variety of factors, including experience, education, and operational need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
