Family Practice - Without OB Physician

Facility is open to provider schedule, but provider must float between locations.

• Worksite Setting : County Jail sites outpatient
• Scope of Work :

10 -15 patients a day & provider must float between three locations near Sacramento.

• Always have MA & security,
• male & female in both locations.
• 100% adult coverage.
• Provider could do intakes, work in detox, withdrawal, or psyche area.
• Care team model with NO CALL.
• Licenses, Certifications, Requirements :

Must have CA licensure, DEA, NPI, BLS,

• Board certified only no exceptions,
• No Malpractice
• NO NPDB issues.
• Must submit CMES
• EMR : Athena
