You'll meet patients in their homes or senior communities, assess their needs, and begin services with clarity and care.\n\nYour Impact Includes : \n

• Completing admissions in homes or facilities across the metro\n
• Answering tough questions with empathy and expertise\n
• Collaborating closely with our care coordination and clinical teams\n
• Documenting accurately and promptly\n\nIdeal Background : \n
• RN, licensed in MN\n
• Hospice, oncology, or palliative care experience required\n
• Strong communicator and natural educator\n
• Clinical confidence and emotional maturity\n
