Requirements
Key Responsibilities
Perform thorough psychiatric evaluations and ongoing mental health assessments for residents in long-term care settings.
Formulate tailored treatment plans that reflect each patient’s specific mental health needs and diagnoses.
Oversee and adjust medication regimens based on clinical response and best-practice guidelines.
Partner with interdisciplinary professionals—including physicians, therapists, social workers, and nursing staff—to ensure coordinated and holistic patient care.
Deliver counseling and psychotherapy to residents and support families in understanding care plans.
Provide education to patients and caregivers regarding mental health conditions, available treatments, and ongoing self-care strategies.
Maintain compliant, accurate, and timely documentation within electronic health records.
Engage in case conferences, team meetings, and quality improvement efforts to support clinical excellence.
Qualifications
Active New York State Nurse Practitioner license with certification as a PMHNP .
Ability to work independently in a fast-paced clinical environment and manage a diverse caseload.
Strong skills in psychiatric diagnosis, treatment planning, psychopharmacology, and patient assessment.
Excellent communication and relationship-building abilities with patients, families, and colleagues.
Proficiency in EHR systems and general computer use.
