Physical Therapist - Home Health - (PT - HH)

StartDate : 6 / 25 / 2024 Available Shifts : 8 D Pay Rate : $1782.90 - $2054.22

PT needed ASAPBLS, COVID Vaccine Required, Pandemic Licensure Approved, Rapid Response Protocol ApprovedM-F 8 : 30a-5p. Mileage is billable per GSA guidelinesEMR HomeCare Home Base (HCHB) experience preferred.

Home Health experience including OASIS required.2 years experience in a healthcare setting required Community / home health experience ispreferred.

Required certs : BLS & NPI # Also needs a CA PT license HH requires a current driver's license & proof of automobile insurance.

Mileage is paid her GSA guidelinesGood oral and written communication skills; has good organizational skills.Profiles go directly to the unit manager for considerationModules are required to be done before start of assignment, time spent on the modules are not billable.

Modules take on average 3-4 hours to complete.

Required Qualifications

CA License required

BLS, COVID Vaccine Required, Pandemic Licensure Approved, Rapid Response Protocol Approved

Home Health experience including OASIS required.

2 years experience in a healthcare setting required Community / home health experience is

preferred.

Required certs : BLS & NPI # Also needs a CA PT license

HH requires a current driver's license & proof of automobile insurance.

Mileage is paid her GSA guidelines

Good oral and written communication skills; has good organizational skills.

Preferred Qualifications

EMR HomeCare Home Base (HCHB) experience preferred.
