We are seeking a dedicated Behavioral Health Professional to become a member of our diverse care team! In this role, you will work as a behavioral health therapist in a school based clinic.

Key Responsibilities :

• Provide comprehensive assessment and diagnosis of Behavioral and Mental Health clients.
• Develop effective treatment plans, that include specific goals to be met and timeline for meeting goals
• Assist clients in achieving established self-management goals
• Evaluate crisis situations and apply appropriate interventions or referrals if needed
• Actively participate in meetings that support an integrated Medical / Mental Health care model to provide comprehensive care for clients
• Assist in the detection of at risk patients and development of plans to prevent further psychological or physical deterioration.
• Assist in the management of chronic disease by working with patients in self-management of chronic diseases
• Refer patients for other treatment when objective progress in patients condition cannot be documented
• Provide trainings for staff and / or families on Behavioral Health issues as needed
• Must comply with HIPAA, HITECH, and Part II regulations.
• Other duties as assigned

Qualifications :

Licensed Clinical Social Worker with current Kentucky license

Must maintain compliance with licensure board continuing education requirements

A minimum of 2 years of direct clinical experience

Integrated medical care model experience preferred
