Summary :
Registered Nurse Breast Care Patient Navigator serves as a single point of contact for patients, caregivers and physicians to provide resources and assistance with accessing clinical and supportive care services offered within the health care system and in the community.
This Job is an advocate for the patient in their dealings with the healthcare community, helping patients navigate their way to timely and quality care, assisting the patient throughout their diagnosis and treatment and aiding in overcoming any barriers to care.
Responsibilities :
- Meets expectations of the applicable OneCHRISTUS Competencies : Leader of Self, Leader of Others, or Leader of Leaders.
- Focus on the clinical aspects of patient care by helping patients in understanding their diagnosis, treatment options and the resources available.
- Help patients navigate their way to timely and quality care from suspicious findings to treatments, while addressing tangible barriers to patient care.
- Serve as an essential link between patients and all other care providers and follow patient through continuum of care.
- Facilitate appointments for consults and support services within established standards.
- Function as a member of an interdisciplinary team to assess, secure and coordinate appropriate services.
- Secure and execute financial grants from state and or national programs if applicable and assure compliance.
- Work with Cancer Registry staff and others to collect data, track outcomes and support strategic work plans.
- Develop educational tools for patients and providers as needed.
- Responsible for educational outreach efforts to community and to key customers (physicians, office staff, diagnostic staff, etc.
to establish and maintain positive working relationships.
Perform all other duties as assigned.
Requirements :
- Education / Professional Qualifications : High School diploma or equivalent required. Bachelor’s degree in Nursing
- Requires problem solving, decision making and critical thinking skills
- Requires excellent leadership, organizational, written and verbal communication and interpersonal skills.
- Computer literate with software office skills plus basic computer knowledge.
- More than 3 years of nursing experience.
- Experience in oncology or case management or nursing specialty is a plus.
- Active State Licensed BSN required.
Work Schedule :
8AM - 5PM Monday-Friday
