Practice Setting : Outpatient, O / R, Inpatient, Clinic, Rounds

Documentation System / EMR : EPIC

Trauma Level / Designation : Level 1

Estimated Credentialing Timeframe : Minimum 60 days

• Board Certified or Board Eligible - Required
• PALS - Required
• Active OH license - Required (will accept providers willing to obtain)
