Registered Nurse\nWeekend Doubles - Saturday and Sunday, 16 Hour Shifts\n32 hours a week - Paid for 40 hours a week\n2 : 45 p.m. - 11 : 15 p.m. and 11 : 15 p.m. - 7 : 15 a.m.\nPay Rate : $34.00 - $42.00 / hour + shift diff\n\nSUMMARY : \n

  • Provide the highest degree of quality nursing care to the residents following Fahrney-Keedy's policies and procedures.\n
  • As requested, supervise the day-to-day nursing activities in accordance with current Federal, State and local standards, guidelines and regulations.\n
  • Monitor the CMA's and GNA's assigned to the floor to ensure the residents receive quality care.\n\nDELEGATION OF AUTHORITY : \nAs an LPN / RN, you are delegated the administrative authority, responsibilities and accountability necessary for carrying out your assigned duties.\n\nEMPLOYMENT AT WILL : \nEmployment with Fahrney-Keedy is "at will". The omission of specific statements of duties does not exclude them from the position if the work is similar, related, or is an essential function of the position.\n\nESSENTIAL DUTIES & RESPONSIBILITIES : \nRatings : \n3 = Exceeds Expectations\n\n2 = Meets Expectations\n\n1 = Needs Improvement\n\nAnnual\n\nEvaluation\n\nDate : \n / / \n\nNotes : \nMake rounds at the beginning and end of shift.\n\nPerform Admission Assessments on new and re-admissions.\n\nConsult with a resident's physician in providing the resident's care, treatment and rehabilitation as necessary.\n\nCreate and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift.\n\nReport all complaints and grievances made by a resident immediately.\n\nParticipate in and receive the nursing report upon reporting for duty.\n\nFollow established policies concerning exposure to blood / body fluids.\n\nReview each resident's records for specific treatment, medication orders, diets, etc.\n\nReview lab reports and notify physicians of results to determine appropriate follow-up.\n\nNotify and provide an accurate assessment to the attending physician when a resident has an accident or incident or when there is a change in the resident's condition.\n\nNotify resident's family and / or responsible party when a resident has an accident or incident or when there is a change in the resident's condition.\n\nAdminister professional services such as medication administration, catheterization, tube feeding, suctioning, applying and changing dressings / bandages, packs, colostomy and drainage bags, taking vital signs, transferring residents, feeding, toileting, etc. as required. Document the provision of services accurately and on a timely basis.\n\nObtain sputum, urine, stool and other specimens for lab tests as ordered.\n\nMonitor seriously ill residents as necessary\n\nAssess resident's skin for signs of breakdown and take immediate action for treatment when indicated.\n\nInstitute and maintain preventative measures for skin breakdown.\n\nEnsure that residents with decubitus ulcers receive appropriate treatment such as daily inspection of wounds turning and positioning, nutritional support and maintaining a clean / dry bed.\n\nEnsure that the GNA's and CMA's are providing direct care to the residents and are providing such care in accordance with each resident's care plan and wishes.\n\nMeet with residents and / or family members as necessary.\n\nInform the family member of the death of a resident; contact the funeral home when requested by the family; follow policy for postmortem care.\n\nMedication Administration\n\nRatings : \n3 = Exceeds Expectations\n\n2 = Meets Expectations\n\n1 = Needs Improvement\n\nAnnual\n\nEvaluation\n\nDate : \n / / \n\nNotes : \nPrepare and administer medications as ordered by the physician within the specified time frames and according to established policy.\n\nDocument acceptance and / or refusal of medications per policy.\n\nAdminister medications orally, topically, by feeding tube, intramuscularly, subcutaneously or rectally as specified per physician's order.\n\nAdminister all physician ordered nourishments.\n\nAssess and observe residents for drug interactions and adverse reactions.\n\nAssess need for PRN medications; assess and record effectiveness of administered dose.\n\nVerify the identity of the resident before administering medication.\n\nEnsure that prescribed medication for one resident is not administered to another.\n\nEnsure that an adequate supply of medications for assigned residents, floor stock medications, supplies and equipment are on hand to meet the nursing needs of the residents.\n\nMaintain medicine cart in a neat, clean and orderly manner.\n\nOrder prescribed medications, supplies and equipment as necessary and in accordance with Fahrney-Keedy and pharmacy policies.\n\nCount narcotics accurately for the shift and document.\n\nDispose of drugs and narcotics as required and in accordance with established procedures.\n\nTreatment Administration\n\nRatings : \n3 = Exceeds Expectations\n\n2 = Meets Expectations\n\n1 = Needs Improvement\n\nAnnual\n\nEvaluation\n\nDate : \n / / \n\nNotes : \nPerform sterile and clean dressing changes as ordered and per facility policy; label, date and initial dressings.\n\nAssess wound appearance, stage, drainage, size and document on skin grid per policy.\n\nAdminister tube feedings per physician's orders (continuous or bolus) and flushes per physician's order and policy.\n\nInsert / change urinary catheters (Foley and suprapubic) per physician's orders and policy.\n\nIrrigate Foley catheters per physician's orders and policy.\n\nInsert / change gastric tubes per physician's orders and policy.\n\nAdminister oxygen and respiratory treatments.\n\nChange oxygen tubing and other respiratory equipment per policy.\n\nPerform Colostomy Care as ordered.\n\nAdminister douches and enemas as ordered.\n\nAssess and document resident's response to treatments and procedures.\n\nCharting & Documentation\n\nRatings : \n3 = Exceeds Expectations\n\n2 = Meets Expectations\n\n1 = Needs Improvement\n\nAnnual\n\nEvaluation\n\nDate : \n / / \n\nNotes : \nComplete required electronic documentation upon resident's admission, transfer and discharge.\n\nEnsure that attending physicians review, record and sign their progress notes and orders in accordance with policies.\n\nReceive telephone orders from physicians and record accurately.\n\nTranscribe physician's orders to residents' electronic charts, MAR, TAR, ADL and Lab books accurately.\n\nElectronically chart nurses' notes in an informative and descriptive manner that reflects the care provided to the resident as well as the resident's response to care. Review previous nurses' notes to assure continuity and follow-up.\n\nElectronically chart all reports of accidents, incidents and change of condition.\n\nComplete incident report for all accidents and injuries – witnessed or unwitnessed; initiate unwitnessed injury investigation when indicated.\n\nElectronically record new or changed diet orders and forward information to Dietary Department.\n\nReport all discrepancies noted concerning physician's orders, diet changes, charting errors, etc. Complete Medication Error Reports as indicated.\n\nElectronically document medication and treatment administration and refusal and response to drugs per facility policy. Report refusal of medications to physicians.\n\nNotify the scheduler when you will be late or absent from work – at least two hours prior to your shift start time.\n\nReport work-related injuries or illnesses and / or occupational exposures to blood, body fluids, infectious materials and hazardous chemicals to the supervisor.\n\nReport known or suspected incidents of fraud to the supervisor.\n\nAdministrative Functions\n\nRatings : \n3 = Exceeds Expectations\n\n2 = Meets Expectations\n\n1 = Needs Improvement\n\nAnnual\n\nEvaluation\n\nDate : \n / / \n\nNotes : \nParticipate in employee performance evaluations, providing information on the performance of GNA's and CMA's to the Director of Nursing.\n\nDevelop and maintain a good working rapport with intradepartmental and interdepartmental staff to ensure that Nursing Services can be adequately maintained to meet the needs of the residents.\n\nAgree not to disclose assigned user ID code and password for accessing resident / facility information and promptly report suspected or known violations of such disclosure to the supervisor.\n\nAgree not to disclose resident's protected health information and promptly report suspected or known violations of such disclosure to the supervisor.\n\nReport any known or suspected unauthorized attempts to access the facility's information system to the supervisor.\n\nEnsure that departmental computer workstations left unattended are properly logged off or the password protected automatic screen-saver activates within established facility policy guidelines.\n\nSafety & Infection Control\n\nRatings : \n3 = Exceeds Expectations\n\n2 = Meets Expectations\n\n1 = Needs Improvement\n\nAnnual\n\nEvaluation\n\nDate : \n / / _\n\nNotes : \nObserve and follow established infection control policies and procedures; ensure that staff follows procedures and that par levels of personal protective devices are available.\n\nEnsure that the nursing unit is maintained in a clean and sanitary manner.\n\nEnsure that residents' rooms and common areas are maintained in a clean, safe, sanitary manner.\n\nParticipate in all fire safety and disaster preparedness drills in a safe and professional manner.\n\nFollow established safety regulations for the use of equipment and supplies and ensure that staff follow established policies for their use.\n\nReport broken or damaged equipment and supplies and complete online TELS work orders as indicated.\n\nParticipate in Fall Risk Management program and other specialty programs and ensure GNA's and CMA's are aware of residents at risk.\n\nInstitute appropriate infection control procedures when residents require specific precautions and / or isolation.\n\nPractice appropriate handwashing and use of hand sanitizer at all times.\n\nWORKING ENVIRONMENT : \nThe work environment characteristics described here are representative of those an associate may encounter while performing the essential duties of this position.   • Honor the resident's refusal of treatment request. Report such requests to your Supervisor.\n\nSPECIFIC REQUIREMENTS : \n
  • Must be able to read, write, speak and understand the English language.\n
  • Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.\n
  • Must be a supportive team member, contribute to and be an example of team work and team concept.\n
  • Must possess the ability to make independent decisions when circumstances warrant such action.\n
  • Must possess the ability to deal tactfully with staff, residents, family members, visitors, government agencies / personnel, and the general public.\n

  • Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices.\n

  • High School Diploma or GED and completion of an approved nursing program. Must be knowledgeable of nursing and medical practices and procedures as well as laws, regulations and guidelines that pertain to long-term care.\n\nPRIOR / RELATED EXPERIENCE : \n
  • Must be an LPN in accordance with the laws of the State of Maryland.\n
  • Must be an RN in accordance with the laws of the State of Maryland.\n\nPHYSICAL & SENSORY REQUIREMENTS (With or Without the Aid of Mechanical Devices) : \n
  • Must be able to move intermittently throughout the workday.\n
  • Must be able to speak and write the English language in an understandable manner.\n
  • Must be able to cope with the mental and emotional stress of the position.\n
  • Must be able to see and hear or use prosthetics that will enable these senses to function adequately to ensure that the requirements of this position can be fully met.\n
  • Must function independently and have flexibility, personal integrity, and the ability to work effectively with residents, staff, and support agencies.\n
