Job Title : Care Coordinator
Job Summary :
A healthcare organization in Birmingham, AL is seeking a highly organized and compassionate Care Coordinator to join their Clinical Services Division. This role is responsible for coordinating patient care activities, managing clinical referrals, verifying benefits, and supporting utilization review and quality assurance processes. The ideal candidate will have a master's degree in psychology or a related field, active licensure, and at least five years of experience in the mental health field.
Principal Duties & Responsibilities
Here's What Your Day-to-Day Looks Like :
- Coordinate care management activities for managed care, employee assistance, and workers' compensation beneficiaries system-wide.
- Communicate with hospitals, outpatient providers, physicians, and patients to ensure quality and continuity of care.
- Receive and manage patient and provider calls, schedule assessments, and monitor case progress.
- Verify benefits and explain coverage details to patients, providers, and client representatives.
- Participate in utilization review and quality assurance activities with the physician review panel and Medical Director.
- Maintain accurate clinical documentation, adhere to HIPAA and URAC standards, and support data analysis and claims review.
- Serve as a clinical liaison for Employee Assistance Program (EAP) accounts, supporting reporting and client communication.
- Provide after-hours coverage for emergencies on a rotating basis.
- Collaborate with Provider Relations to facilitate facility transfers or emergency care coordination.
- Maintain departmental files, assist with goal achievement, and stay current through continuing education.
Schedule & Pay
Monday – Friday, 8 : 00 AM – 5 : 00 PMSalary : $66,000 – $70,000 annuallyAnnual Incentive : Up to $4,600 – $6,000 (based on base salary and performance)On-siteQualifications
