Job Summary:
The Wayne Mobile Health Unit Family Medicine Nurse Practitioner provides medical services in
various locations. The NP travels with the WMHU to various locations including community centers,
churches, businesses and patients' homes. The NP services include assessing, diagnosing and treating
illnesses, prescribing medications, providing preventive care, education, counseling, collaboration,
and support.
Job Description:
Essential Duties and Responsibilities:
• Perform assessment by obtaining patient medical and social histories
• Select and order appropriate diagnostic and health maintenance procedures
• Plan for appropriate follow-up care
• Evaluate the signs and symptoms of the patient.
• Interpret and evaluate diagnostic tests and laboratory results.
• Initiate treatment and manage patient treatment plans for acute and chronic illnesses.
• Monitor and review patient care and treatment plans, which include nutrition and exercise plans.
• Provide training and clinical support to on-site personnel.
• Promote health education and wellness.
• Supervise and/or administer required tests, immunizations, injections, and medications.
• Establish and revise written medical protocol in collaboration with physicians.
• Refer patients to physicians or other health care providers and facilities as needed and monitor
the outcome of the referral.
• Consult and collaborate with other professionals as needed.
• Provide on-call coverage for evenings and nights as needed.
• Provide physical examinations on patients per protocols
• Mediate pharmacological interventions
• Carry out preventative health assessments, screening, immunizations, and patient care
• Counsel and educate patients and their families/caregivers concerning preventative health,
treatment options, and community resources
• Attend staff meetings
• May be called upon to administer Social Determinants of Health (SDOH) Questionnaire while at
WMHU events.
• Participate in the teaching of medical students, residents, and other students.
• Participate in quality assurance activities.
• Maintain the privacy and confidentiality of patient information
• Other duties as assigned
