Your Impact as a Clinical Case Manager

In this role, youll support our Clinical Team in guiding individuals on their recovery journey. From intake and initial assessments to treatment planning and discharge summaries, youll play a key role in ensuring our clients receive the highest level of care.

What Youll Do :

Conduct biopsychosocial assessments and develop treatment plans

Collaborate with professionals to create effective recovery strategies

Assist with documentation, scheduling, and tracking client progress

Engage in ongoing education and professional growth

Help individuals build happier, substance-free lives

What You Bring :

Bachelors degree in a related field

Experience in mental health & substance use disorders

Strong time management, organization, and communication skills

Passion for working in a fast-paced, dynamic environment
