Job Summary :

Assesses and coordinates the training and educational needs of healthcare professionals in an assigned area to support staff development and performance improvement.

Education & Experience

Education and Experience Required :

Degree (Associate's or Bachelor's) and two (2) years of experience in healthcare, education, or program coordination.

Master's degree in Social Work, Public Health, or related field preferred.

Minimum 3-5 years of experience in care coordination, case management, or healthcare education preferred.

Certifications, Licenses, or Registration preferred :

ACMA or CCM Certification preferred

LCSW preferred

Knowledge, Skills & Abilities
