• Follow the interdisciplinary plan of care, be alert to the patient's needs and concerns
• Provide personal care, including bathing, dressing, grooming, hair, nail, skin and foot care and oral hygiene
• Assist patients, according to the plan of care in : Turning, Sitting, Standing.
• Ambulation with elimination, including : facilitation of bedpans, and urinals. Assisting patient to the bathroom. Routine Foley care-Enemas, Meals, when necessary.
• Perform simple procedures as an extension of therapeutic services, in accordance with the interdisciplinary plan of care
• Observe and report changes in the patient's condition and needs to the Team Director (TD) or Registered Nurse (RN)
• May perform simple homemaking chores for the patient directly, meal preparation, and / or light housekeeping in the patient's immediate area
• Complete required documentation per policy
• Participate in continuing education programs
• Follow confidentiality regarding patients and families per policy
• Demonstrate a compassionate attitude toward patients and families consistent with the philosophy and mission of the organization
• Attend patient care conferences as requested by the TD
• Perform other related duties and special projects, as assigned
• Perform other duties as assigned including those within the scope of current, valid professional licensure / credentials.
