Key Responsibilities

Provide monthly care coordination through planning, facilitation, and advocacy for patient health needs

Identify patient-specific problems, goals, and interventions based on clinical assessments

Maintain patient chart compliance through accurate documentation and updates

Required Qualifications

Current license and registration as a Licensed Practical Nurse in a COMPACT state

LPN degree from an approved program is required

Successful completion of a background check
