- Perform thorough patient assessments, identifying important health changes and planning individualized care in collaboration with the healthcare team.\n
- Develop and adjust patients' care plans based on ongoing evaluations and patient needs, ensuring smooth transitions during discharge.\n
- Provide compassionate direct care and education, respecting the cultural and spiritual needs of patients and their families.\n
- Delegate appropriately and supervise nursing tasks performed by other team members, ensuring compliance with the care plan.\n
- Maintain a safe patient environment while continuously monitoring quality and effectiveness of care.\n
- Engage in performance improvement initiatives, contributing to enhanced patient care outcomes.\n
- Advocate for patients and coordinate care, ensuring a seamless approach within the healthcare continuum.\n
- Document all nursing care accurately to support the achievement of patient / family goals.\n
- Participate in ongoing education to maintain your nursing competence and licensure.\n\nSupervisory / Management Responsibility\n
- This is a non-management role reporting to a supervisor, manager, director, or executive.\n\nMinimum Requirements\n
- Education : Associate degree in Nursing (Bachelor's preferred).\n
- Experience : One year of experience as a registered nurse.\n\nIn Lieu Of\n
- Successful completion of Prisma Health Nurse Residency Program or similar residency may substitute for the experience requirement, or one year of LPN experience in the same service line for internal candidates.\n\nRequired Certifications, Registrations, Licenses\n
