The Bridge Family Center is looking for a Case Manager to work with young adults in its Independent Living Program. This position would work with the Community Housing Assistance Program (CHAP) program, working with DCF involved clients. This is a full time position with some schedule flexibility to meet the needs of the clients. This position requires the employee to travel locally within CT to work with clients. Employees may have access to a shared agency vehicle, but may need to use their own vehicle, as well. Mileage reimbursement is available.

GENERAL STATEMENT OF DUTIES : The Case Manager for CHAP is responsible for monitoring all aspects of their assigned cases. Case Managers are involved with case management, life skills instruction, and program management for at-risk adolescents.

Qualifications : Bachelor's Degree in Human Services or related field, and experience working with adolescents. Must have the ability to set professional boundaries and limits, cultural sensitivity, excellent communication and interpersonal skills, ability to take direction from supervisor, and ability to establish positive rapport with clients. Requires the ability to organize responsibilities and function independently. Must also have a valid Connecticut driver's license, a reliable form of transportation and ability to transport clients, and possess the following certifications, or be eligible to obtain these after hire : LIST Training and First Aid / CPR.
