NOW HIRING : Hospital Nurse Liaison Responsibilities Development, generating and maintaining hospital relationships with CMs / SWs / Physicians Assists with patient eligibility for Skilled Nursing and Long-Term Care by performing in-person assessments-when able-reviewing medical records and coordinating with facility admissions department. Responds appropriately and completely to incoming referrals in a timely manner. Communicate with patients and their families throughout the admission process. Interacts with a variety of hospital staff from different departments. Coordinates with facility staff to ensure that each patient has a smooth transition through the admission process. Qualifications Knowledge and experience with marketing strategies, goals and plans Knowledge of Basic Computer skills and ability to learn new programs. Knowledge of health insurance coverage for patients.
