Position Summary :

The Intake & Referral Specialist plays a critical role in the onboarding of new clients and coordination of referrals. This position requires strong communication skills, a deep understanding of home health services, and the ability to work collaboratively with healthcare providers, patients, and internal teams.

Key Responsibilities :

• Receive and process referrals from physicians, hospitals, and other healthcare providers.
• Conduct initial intake assessments to determine eligibility and service needs.
• Coordinate with clinical staff to ensure timely initiation of care.
• Maintain accurate documentation and records in compliance with regulatory standards.
• Communicate effectively with clients and families to explain services and answer questions.
• Collaborate with insurance providers and verify coverage.
• Ensure smooth transitions from referral to active service.

Requirements

Qualifications : Minimum 1-3 years of experience in a home health or healthcare intake / referral role. Some understanding of Medicare, Medicaid, and private insurance processes. Excellent organizational and multitasking skills. Proficient in Microsoft Office and electronic health record (EHR) systems. Bilingual in Spanish is preferred, but not mandatory. High school diploma required; associate or bachelor’s degree in healthcare administration or related field preferred.
