Position Overview

Join our Inpatient Palliative Care Team at Optum , focused on providing compassionate, clinically excellent care to seriously ill patients. The role centers on helping patients and families understand their medical future and aligning care with their goals.

Key Responsibilities

• Conduct initial consults and follow-up visits for hospitalized patients.
• Guide patients through prognosis and care planning.
• Manage symptoms and oversee treatment plans.
• Collaborate with physicians, nurses, social workers, and chaplains.
• Participate in daily huddles, bedside family conferences, and interdisciplinary rounds.
• Document care accurately in Epic EMR .
• Support healthcare providers dealing with patient distress.
• May involve teaching residents and students.
