Case Manager - Scattered Site

This position is responsible for the delivery of comprehensive, intensive, clinical-based case management for adults experiencing chronic homelessness involved in the Housing First and Assertive Community Treatment (ACT) services program. Services are based on motivational interviewing, stages of change, strengths-based case management, housing first, and trauma-informed care. The position will provide outreach services, referral and linkage with mainstream services and housing, and crisis intervention for program clients. Additional Requirement
