The Case Manager will be an integral part of our healthcare team, providing comprehensive and compassionate case management services to clients. Your role involves collaborating with healthcare professionals and others as needed to address our clients’ medical, social, and emotional needs of our clients, ensuring they receive the highest quality of care.

Essential Job Duties

KNOWLEDGE REQUIRED :
Required and preferred knowledge and experiences to succeed.
Bachelor’s degree required, or a minimum of four years of directly related experience may be considered in lieu of a degree; except in Wisconsin where a bachelor’s degree is mandatory.
Previous experience in case management preferred.
Ability to learn multiple databases and software systems.
Strong interpersonal and communication skills.
Salary :
$49,920-$61,102 / annually (Depending on Experience)
