About The Position

Must-Have :

• Two (2) years of experience in a home health setting within the last ten (10) years
• Recent experience conducting comprehensive assessments of patients

Description :

Required licensure includes current RN Licensure in Client.

Two years of experience in a home health setting within the last ten years is mandatory.

This experience may include working in the field for a Certified Home Health Agency, a Licensed Home Care Service Agency, or Managed Long-Term Care.

Candidates must have experience conducting assessments similar to UAS such as OASIS and providing primary care to homebound individuals.

Care management for persons receiving home and community-based services is also important.

Home care experience as a paraprofessional or Licensed Practical Nurse caring for at least two patients per week is preferred.

In addition to required qualifications, preferred qualifications include experience conducting assessments in UAS-NY and caring for a geriatric population.

The ideal candidate will have experience managing chronic diseases in a home setting.

Home care experience as a paraprofessional or LPN combined with RN experience is highly valued.

Candidates with limited experience in home care may not qualify for this position.

Assessment experiences conducted solely telephonically will not fulfill the required qualifications.

Candidates should be prepared for a role that involves both hybrid tele and in-field assessments.

The responsibilities include performing initial in-home evaluations of patients, reviewing medical documentation, and entering data into electronic forms.

Documenting concerns and facilitating communication with interdisciplinary care teams is essential.

Maintaining knowledge of community resources and care coordination will aid in improving service delivery.

Candidates must be able to work in a fast-paced, deadline-oriented environment and manage multiple complex tasks simultaneously.

An understanding of health promotion and illness prevention will be beneficial.

Additional duties may be assigned by management as necessary.
