Description :
The Integrated Case Manager for Population Health is an interdependent member of the patient-centered care team.
Responsible for the collaborative practice of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services.
Addresses the comprehensive healthcare needs of individuals and families through communication and available resources.
Promotes patient safety, quality of care, and cost-effective outcomes.
Conducts a comprehensive assessment of patients and family / caregiver biomedical, psychological, social, and functional needs.
Develops personalized patient-centered care plans aimed at optimizing the patient’s care experience.
Engages patients and their families as part of the care team through advocacy and ongoing communication.
Utilizes professional judgment, critical thinking, and self-management techniques to assist patients in overcoming barriers to goal achievement.
Provides counseling and interventions related to treatment decisions and end-of-life issues, including Advanced Care Planning.
Ensures patients transition seamlessly and safely between care settings.
Advocates for the appropriate delivery of services within the patient’s health plan benefit structure.
Collaborates with members of the treatment / care team to co-manage patients with complex medical and social needs.
Continually evaluates the patient’s response to the care / treatment plan, making modifications when necessary.
Participates in process improvement activities designed to reduce risk, including data collection and analysis.
Facilitates interventions in cases involving abuse or violence.
Supports department-based goals that contribute to the organization's success.
Performs other duties as assigned.
Excellent verbal communication and written documentation skills are essential.
Strong problem-solving, analytical, and decision-making skills are required.
Knowledge of preventive service guidelines, Medicare and Medicaid regulations, and case management principles is preferred.
Understanding of social determinants of health and their impact on patient well-being is necessary.
Strong organizational, planning, and implementation skills with the ability to handle multiple complex patient needs simultaneously are required.
A strong sense of compassion with the ability to successfully advocate for patients and their families is crucial.
