Job Summary

Under minimal supervision, performs technical Public Health work in the detection and treatment of communicable diseases. Responsible for working with, testing and counseling persons exposed to, infected with, or having a positive test for communicable diseases in a variety of clinical, community, business, institutional, and home settings. Work also involves providing information about the disease and course of treatment, ensuring care plan adherence, providing appropriately prescribed treatment and/or prophylaxis, collecting and reporting medical, psychological and social information to physicians and nurses, health departments and laboratories, and facilitating the linkage of clients to appropriate staff for additional care and services as required .

The Disease Intervention Specialist will be part of the Tuberculosis Clinic, one of the four clinics within the Public Health Institute at Denver Health. We are looking for someone with an understanding of social justice, health equity, and systemic barriers that marginalized communities face to join our team. The Public Health Institute at Denver Health (PHIDH) strives to understand and acknowledge systemic and structural barriers that perpetuate racial inequities in health outcomes.

Phlebotomy certification and a Medical Assistant Training Program is strongly preferred.



Essential Functions :

  • Field Work and Clinic Setting:
  • TESTING
    â—¦ Triages, registers, tests, delivers test results and provide crisis counseling or brief therapy techniques, documented by completion of appropriate in-service training.
    â—¦ Uses standard precautions for specimen acquisition and storage, including phlebotomy.
    â—¦ Uses basic laboratory techniques necessary for the successful implementation and interpretation of clinical tests.
    â—¦ Interpret test results according to current clinical standards and explain results to the patient accurately and appropriately for that individual, including conveying difficult information about serious and complex illnesses.
  • COUNSELING, EDUCATION, AND FOLLOW-UP
    â—¦ Possesses a working knowledge of diseases/conditions specific to the area and interventions related to them.
    â—¦ Conducts pre- and post- test counseling to clients who present for testing.
    â—¦ Assists patients to identify barriers to effective risk reduction and health promotion.
    â—¦ Works with clients to identify modifiable behaviors and other issues impacting health and have ability to refer them to available resources to help with these issues.
    â—¦ Facilitates client access and transition to follow-up services as indicated.
    â—¦ Provides referrals for Denver residents to the Medicaid, CICP, DFAP, and Child Health Plan program enrollment staff.
    â—¦ Participates in care coordination, extended to settings outside the clinic that require good professional working relationships with various community based organizations and agencies serving the same clientele.
    â—¦ Accurately and clearly articulates to clients, health care staff, community members, organizations, and businesses basic public health principles and standard of care to protect public health and to prevent the spread of specific communicable diseases.
    â—¦ Recognizes when assistance from other staff is needed for the implementation of treatment, interpretation of tests, and follow up care.
    â—¦ Readily recognizes and reports problems or untoward events related to testing, medication and/or treatment.

  • Accurately and appropriately documents all patient encounters, including entry of required data into medical records and other tracking programs, including:
  • Uses of electronic resources and maintain data bases to identify individuals requiring coordinated outreach and intervention from multidisciplinary team.
  • Collects, organizes and analyzes data to perform specific Public Health activities for communicable disease prevention
  • Articulates and presents findings and data through verbal, written and electronic communication

Education :

  • Associate's degree Required or High school diploma with 2 years of directly related experience in community or clinical health experience in addition to experience requirements below


Work Experience :

  • Three years relevant experience. Required
  • Working with patients or an equivalent combination of education and experience required


Licenses :

  • Phlebotomy certification and a Medical Assistant Training Program is strongly preferred.


Knowledge, Skills and Abilities :

  • Bilingual/Spanish preferred.
  • Phlebotomy skills required within 6 months of start date.
  • Must have exceptional organizational skills and very strong interpersonal skills.
  • Computer skills including Word, Excel and Power Point preferred.
