Nurse Practitioners work in our outpatient clinics as part of a collaborative care team that includes physicians, PAs / NPs, medical assistants, scribes, and nursing support. With access to top specialists and hospitals, providers can focus on in-clinic primary care, care coordination, and population health.
Core Responsibilities :
- Deliver high-quality primary care and manage routine and acute visits.
- Perform annual wellness exams and health assessments with thorough, accurate documentation.
- Coordinate care with specialists, facilities, and community agencies.
- Support population health efforts with the Care Team.
- Assist with phone triage, patient outreach, and health education.
- Participate in clinic and community promotional activities.
- Conduct home visits when needed and perform other assigned duties.
Required Qualifications :
MSN, DNP, or NP certificate graduateNational NP certification (AANP or ANCC)Active, unrestricted NP license and DEA registrationAuthorization to work in the U.S.Preferred Qualifications :
