A company is looking for a Well-being Navigation Care Guide.

Key Responsibilities

Acts as the central facilitator to patients, supporting them through all stages of care

Identifies barriers to healthcare goals and assists patients in accessing resources

Develops patient-driven goals and supports adherence to care plans

Required Qualifications

Must be 18 years of age with relevant education and / or experience

0 to 2 years of experience in customer service, health care, or community-based roles

High school diploma or GED preferred

Bachelor's degree in health care is a plus
