Licensed Practical Nurse - ACT Team - Bronx, NY (Onsite) - Full Time - Mon-Fri 09 : 00am-05 : 00pm or 08 : 00am-04 : 00pm - $60,000 Per Year plus 2k for Anyone Bilingual in Spanish

A leading nonprofit supporting individuals and communities through mental health, social services, and recovery programs is seeking a full time Licensed Practical Nurse (LPN) for their ACT team. Seeking someone passionate about making a meaningful impact and helping every person reach their full potential.

Location : Bronx, NY (Onsite)

Employment Type : Full Time, Day Shift, Mon-Fri 09 : 00am-05 : 00pm or 08 : 00am-04 : 00pm

Compensation : $60,000 Per Year + 2k for anyone Bilingual in Spanish

Description :

Make a meaningful difference in the lives of individuals with complex needs.

Collaborate in a supportive team setting.

Utilize clinical nursing skills in community-based care and advocacy.

Participate in recovery and wellness initiatives.

Key Responsibilities :

Conduct health assessments and develop care plans within LPN scope.

Support clients in accessing medical care, housing, and community resources.

Collaborate with RNs and psychiatrists to manage medications and coordinate care.

Administer medications, vaccines, and perform phlebotomy.

Monitor vital signs and report findings.

Maintain accurate documentation in the Electronic Health Record (EHR).

Provide culturally competent care and participate in crisis intervention.

Qualifications :

Licensed Practical Nurse (LPN) in New York State.

1 year experience with mental health, substance use, homelessness, or criminal justice populations preferred.

New Grads Welcome.

Knowledge of community resources and support services.

Strong organizational, communication, and computer skills.

Spanish fluency a plus.
