Patient Care Coach

A company is looking for a Patient Care Coach to support patients in improving therapy adherence and health outcomes.

Key Responsibilities

Provide one-on-one mentoring and coaching to patients on device use and adherence

Monitor patient adherence and usage data, and coordinate follow-ups to address compliance issues

Document all patient interactions and maintain accurate records in compliance with standards

Required Qualifications, Training, and Education

High school diploma or equivalent required; Associate's degree preferred

One year of work-related experience in healthcare or related fields

Experience in health care administrative roles, customer service, or billing preferred

Familiarity with chronic conditions and related therapies

Technical aptitude for device functionality and data analytics
