To support a dynamic clinical pharmacy team, the full-time California Licensed Pharmacy Technician will assist with medication management programs, improve pharmacy quality, and engage in patient outreach while working remotely on a 6-month contract basis. Key responsibilities Support the Pharmacy Team on CMS/HEDIS Quality STAR measures for patients with chronic conditions Conduct outbound and inbound calls to address gaps in care and improve healthcare outcomes under pharmacist supervision Document clinical activities and communicate with healthcare staff regarding clinical intervention programs Required qualifications National Pharmacy Technician Certification (PTCB or NHA) High School Diploma and 2-3 years of pharmacy-related experience Preferred: Experience with CMS/HEDIS Quality STAR measures
