Position Summary
The Licensed Practical Nurse (LPN) provides skilled nursing care to patients in their homes. The LPN supports the execution of individualized care plans designed to promote recovery, maintain health, and ensure patient safety in accordance with physician orders, state and federal home health regulations, and Graceful Aging Home Health Care LLC policies and procedures.
The LPN plays a vital role in the home health interdisciplinary team, delivering quality clinical care while fostering trust, communication, and dignity for each patient.
Key Responsibilities
Clinical Care
- Provide direct nursing services in the home setting, including :
Medication management and setup
Patient educationWound and ostomy careMedication administration and educationCatheter and ostomy managementInjections, IV therapy (if certified), and blood glucose monitoringVital sign assessments and symptom observationImplement the care plan as developed by the Director of Nursing.Monitor, document, and report patient progress, changes in condition, or adverse events promptly to the supervising RN or physician.Reinforce patient and family teaching regarding disease management, medication adherence, and infection prevention.Ensure all clinical tasks are performed in accordance with the Home Health Conditions of Participation and professional standards.Care Coordination
Collaborate closely with the interdisciplinary team-including RNs, therapists, social workers, and physicians-to ensure continuity and quality of care.Participate in case conferences, chart reviews, and interdisciplinary meetings to support optimal patient outcomes.Communicate regularly with patients, families, and caregivers to ensure understanding of care instructions and adherence to the treatment plan.Report updates, concerns, or new findings to the RN in a timely manner.Documentation & Compliance
Complete all clinical documentation accurately and in real time, in accordance with Graceful Aging Home Health Care and regulatory standards.Maintain patient records in the agency's EMR system, ensuring timely submission of visit notes, assessments, and progress reports.Adhere to all infection control policies, HIPAA privacy rules, and professional nursing standards of practice.Participate in quality assurance, performance improvement (QAPI), and compliance activities as assigned.Patient Advocacy & Support
Promote patient independence while providing supportive education and guidance.Advocate for the patient's medical, emotional, and psychosocial needs.Respond promptly and effectively to emergencies, following agency protocols and seeking immediate RN or physician support when needed.Qualifications
Education : Graduate of an accredited Practical Nursing program.Licensure : Current, unrestricted LPN license in the state of employment.Experience : Minimum of one (1) year of nursing experience preferred; home health experience strongly preferred.Requirements :TB test and physical clearance (required prior to start)

Field-based position; travel to patient homes is required.Must have reliable transportation.Physical Requirements
Ability to lift and transfer patients safely (up to 50 lbs).Extended periods of walking, standing, and driving between patient visits.Manual dexterity and visual acuity sufficient to perform clinical assessments and documentation.
