Bachelor of Science in Nursing (BSN) degree, active registered nurse (RN) license and minimum 1+ years RN experience required. Applicants who do not meet these qualifications will not be considered.

An experienced Home Health Registered Nurse (RN) is needed to deliver skilled nursing care in patient homes within Lebanon, OR. This contract position offers a balance of independent fieldwork and collaboration with healthcare teams to ensure high-quality, patient-centered care.

Key responsibilities include conducting OASIS assessments, developing individualized care plans, providing wound care, medication administration, IV therapy, and managing chronic diseases. You will monitor patient progress, educate patients and families on care plans, and maintain accurate documentation using Home Care Home Base (HCHB). Coordination with physical therapists, social workers, and home health aides is essential to promote comprehensive care.

Qualifications or Desired Experience :

• Active RN license in the state of practice (good standing)
• Minimum of 1 year recent home health experience; field experience preferred
• Proficiency with Home Care Home Base (HCHB) and OASIS assessment tools
• Strong clinical assessment, critical thinking, and communication skills
• Reliable transportation, valid drivers license, and auto insurance
• Ability to work independently and handle a flexible, field-based schedule
• BSN preferred
• Knowledge of Medicare and home health compliance standards and experience managing 2532 points per week is advantageous
