Registered Nurse - Care Coordination
Massachusetts Care Coordination Network, an Affiliate of Seven Hills Foundation
The RN is responsible for reviewing the information with the Enrollees, gathered in the
comprehensive assessment; reviewing the Enrollee's care plan; conducting medication reviews and providing oversight of LPN (not more than two) staff employed by MCCN; meeting weekly with each LPN to review difficult cases; providing specialty Care Coordination with other medical
providers; and being available for both telephonic and in-person consultations for acute changes of status in enrollees.
Pay : $33.65 / hour

Provide medical consultation to MCCN team members regarding acute changes in Enrollees' statusServe as liaison to and consult with the MCCN Medical DirectorConduct medication reconciliations during comprehensive assessments and after inpatient, ED, or 24-hour setting dischargesCoordinate with PCPs to verify discharge medications, address discrepancies, and promote adherenceEducate Enrollees on proper medication administration and identify support systems to aid complianceSupport Care Coordinators in ensuring access to appropriate medical and specialty careReview and complete medical sections of Enrollee assessments and collaborate on LTSS componentsParticipate in interdisciplinary team meetings to promote effective communicationAssist with staff orientation and training; provide coaching on medical conditions, risk factors, and CP team prioritiesFollow all MCCN policies, protocols, and evidence-based practices while assisting with staff trainingApply a person-centered approach in planning and delivering care in coordination with Enrollees and their support systemsAddress medical risks and emergencies with appropriate interventions; develop medical protocols as neededComplete all documentation and reporting in a timely and compliant mannerMaintain strong communication with staff, Enrollees, and care team members to foster collaborationEnsure adherence to HIPAA, agency policies, and performance standardsAttend required trainings, staff meetings, and supervision; maintain all certifications and licensesTreat all clients with dignity and respect in alignment with MCCN's Human Rights policyPerform all duties in accordance with agency standards and expectationsQualifications
Education & Experience :
Registered Nurse licensed in the Commonwealth of MassachusettsMinimum of 4 years of experience working with adults in a community-based mental health, substance use and / or physical health settingDemonstrated knowledge base and judgment necessary for independent clinical decision making.Skills and Knowledge :
Excellent verbal and written communications skills. Strong interpersonal and customer relations skills required.Ability to use Electronic Health Records (EHR) Systems to document and coordinate services.Excellent organizational, time management and problem-solving skills.Strong computer knowledge, including proficiency in contemporary Windows operating systems and Windows office suites with an emphasis on Word and Excel; ability to learn new systems; experience entering and working with data; and comfort and experience using mobile technologies.Knowledge regarding psychiatric rehab and understanding of recovery model.Strongly prefer that a candidate will have a demonstrated understanding of and competence of Health Equity and in serving culturally diverse populations.Must be able to perform each essential duty satisfactorily.Must hold a valid drivers' license. Must have access to an operational and insured vehicle and be willing to use it to transport members.Must have ability to read English and communicate effectively in the primary language of the program to which he / she is assigned.Why Join Us?
